For many people, arranging care feels like a maze of desks and forms. It does not have to be. At RytelZorg we guide you from the first phone call to care in your home, and beyond. This is how it works.
1. Registration
You call, send a WhatsApp message or fill in the contact form. We respond as quickly as possible. You can also first take the free care check to see which care and funding suit you.
2. Introduction and care needs
We visit you at home free of charge. In about 30 minutes we discuss what you need, what you can still do yourself and what matters to you. Your family is welcome.
3. Funding
Together we look at which scheme fits: community nursing through your health insurance (Zvw), support through the municipality (Wmo), the Long-term Care Act (Wlz) or a personal budget (pgb).
4. Application
We help you with the application and the forms, so you do not have to chase anything yourself. Useful to have ready: your insurance card, an up-to-date medication overview and any letters from your GP.
5. Care plan
We record the agreements in a personal care plan: which care, on which days and times, and who will come. You read the plan and give your consent.
6. Start of care
Your regular care professionals start at the agreed times. You always see the same faces and have one point of contact.
7. Evaluation
We review the care plan with you regularly. If your situation changes, we adjust the care or help you get a new assessment.
